
As of September 23rd, WorkPartners is the administrator of Family and Medical Leave (FMLA) coverage for Dentsply Sirona’s U.S. employees.
WorkPartners is part of the University of Pittsburgh Medical Center (UPMC) and has provided absence management services for more than two decades. Employees can call WorkPartners at any time. A phone line dedicated to Dentsply Sirona is staffed 24 hours per day, 7 days per week, 365 days per year.
Additionally, WorkPartners has a team of specialists dedicated to Dentsply Sirona, including an intake customer service team, leave specialist, nurse case manager and medical director.
Employees in the U.S. will need to call WorkPartners at:
1-833-DSHR4you (374-7496), and use option 1 for all new FMLA requests, questions related to FMLA cases and to report intermittent absences.
The following events qualify for FMLA leave:
- Birth/Adoption/Placement of a foster child
- Child/Spouse/Parent’s serious health condition
- Own serious health condition
- Family member of a servicemember of the U.S. Armed Forces on active duty or impending call to active duty
- Family member caring for an injured servicemember or veteran
Employees should call WorkPartners at 1-833-DSHR4you (374-7496) for the following:
- New FMLA requests
- Questions related to their FMLA cases
- To report the use of all Intermittent FMLA time
Please be prepared to provide the following information on the call.
- Employee Name and Date of Birth
- Preferred Phone number, Mailing Address and Email Address
- Reason for Leave request
- Name of Family Member (if relevant to request)
- Date of Birth of Child (if relevant to request)
- Absence start date and expected return to work date
